I lost my certificate years ago and sent for a new one at this site (Luckily I still had my wallet sized card I was issued) that had all the info as to date, Lat. & Long. etc.  The charge wasn't very much, $8 or $9 I think. I was pleasantly  surprised when I received it.  It was so much more elaborate than the original.  I immediately framed it and hung it on my little den wall with all the other stuff.
